INSTRUCTIONS: CALIFORNIA INSTALLMENT AGREEMENT FINANCIAL STATEMENT (Form FTB 3561)
If applying for an installment agreement for a payment plan related to California state tax, you will be required to submit a financial statement if you owe more than $10,000 or need more than 36 months to complete payment. This purpose of this form FTB 3561 is to verify you have a financial hardship justifying your request. The form is found on the website of the California Franchise Tax Board.
