The form discussed in this article is used to document other credits not shown on another schedule filed by a California guardianship or conservatorship. This document can be obtained from the website of the California Judicial Branch.

California Other Credits — Standard And Simplified Accounts GC-400(OCR)/GC-405(OCR) Step 1: Check the first box if filing for a conservatorship.
