INSTRUCTIONS: CALIFORNIA ORDER PRESCRIBING NOTICE (PROBATE) (Form DE-200/GC-022)

The form discussed in this article is used by a California court to order who must receive notice regarding certain types of upcoming probate hearings on petitions. This document can be obtained from the website maintained by the California Judicial Branch.

California Order Prescribing Notice (Probate) DE-200/GC-022 Step 1: In the first blank box, enter your lawyer's name, state bar number, address and telephone number. If you are not represented by an attorney, enter your name, address and telephone number. In either case, entering a fax number or email address is optional.

California Order Prescribing Notice (Probate) DE-200/GC-022 Step 2: In the second blank box, enter the county in which the court is located, as well as its street and mailing addresses, city, zip code and branch name.

California Order Prescribing Notice (Probate) DE-200/GC-022 Step 6: Enter the name of the decedent, minor or conservatee. Indicate which with a check mark.

California Order Prescribing Notice (Probate) DE-200/GC-022 Step 8: Check the appropriate box on lines 1a through 1f to describe the type of petition which will be considered at the hearing.

California Order Prescribing Notice (Probate) DE-200/GC-022 Step 10: Indicate with a check mark whether the address of the court is the same as that given above or different. If the latter, specify.

California Order Prescribing Notice (Probate) DE-200/GC-022 Step 11: Check the box on line 2a if the petitioner will be required to personally serve someone with notice. Enter the number of days before the hearing by which this must be done and the names of the persons who must be served.
