INSTRUCTIONS: CALIFORNIA ORDER PRESCRIBING NOTICE (Form DE-200/GC-022)

The form discussed in this article is issued by a California court to prescribe notice concerning the time and place of hearing on a petition related to a guardianship, conservatorship or estate. This document can be obtained from the website maintained by the California Judicial Branch.

California Order Prescribing Notice DE-200/GC-022 Step 1: In the first blank box, enter the name of your attorney, in addition to their state bar number, address and telephone number. If not represented by an attorney, enter your name, address and telephone number. In either instance, providing either a fax number or an email address is optional.
