INSTRUCTIONS: CALIFORNIA LETTERS OF GUARDIANSHIP (Form GC-250)

This document is issued by California courts to appoint a guardian. This form can be found on the website maintained by the California Judicial Branch.

California Letters Of Guardianship GC-250 Step 1: In the first box, enter your attorney's name, State Bar number, address and telephone number. If you do not have an attorney, enter your name, address and telephone number. In either case, you are not required to provide a fax number or email address.

California Letters Of Guardianship GC-250 Step 2: In the second box, enter the county in which the court is located, its street and mailing addresses, its cip and zip code, and its branch name.

California Letters Of Guardianship GC-250 Step 3: In the third box, enter the name of the minor in question.

California Letters Of Guardianship GC-250 Step 4: Indicate with a check mark whether this concerns guardianship of a person or of an estate.

California Letters Of Guardianship GC-250 Step 5: Enter the case number.

California Letters Of Guardianship GC-250 Step 6: On line 1, enter the name of the person being appointed.

California Letters Of Guardianship GC-250 Step 7: Indicate whether they are appointed to guardianship of a person or of an estate with a check mark.

California Letters Of Guardianship GC-250 Step 8: Enter the name of the person or estate.

California Letters Of Guardianship GC-250 Step 9: Check line 2 if other powers have been granted or imposed.

California Letters Of Guardianship GC-250 Step 10: Check line 2a if the powers granted are described separately in attachment 2a.

California Letters Of Guardianship GC-250 Step 11: Check line 2b if conditions relating to care and custody of property are specified in attachment 2b.

California Letters Of Guardianship GC-250 Step 12: Check line 2c if conditions relating to treatment of minor are specified in attachment 2c.

California Letters Of Guardianship GC-250 Step 13: Check line 2d if other conditions or powers are specified below or on attachment 2d.

California Letters Of Guardianship GC-250 Step 14: Check line 3 if the guardian is not authorized to take possession of money or any other property without a specific court order.

California Letters Of Guardianship GC-250 Step 15: Enter the number of pages attached on line 4. The clerk will sign and seal the form where indicated.
