INSTRUCTIONS: PETITION FOR GUARDIAN (GC-210)

“Petition for Guardian GC-210 Step 1”
If you’re using the assistance of an attorney, provide their information in the top of the form. If you’re not using an attorney, leave the section blank.
“Petition for Guardian GC-210 Step 2”
In the next box, provide the county of the Superior Court, along with the address, mailing address, city and zip, and the branch name. Ask the Clerk of Court for help with this section if necessary. The Clerk of Court will fill out the rest of the section.
“Petition for Guardian GC-210 Step 3”
In section 1, check the appropriate box or boxes and provide your full legal name, address, and contact information. The Clerk will fill out part C. of the first section stating whether a bond is needed or not. The clerk will fill out the rest of section 1 as well.
“Petition for Guardian GC-210 Step 4”
Section 2 requires an attachment of the copy for Guardianship Petition—Child Information Attachment (form GC-210(CA)) for each minor. Provide the full name and date of birth for each minor.

In section three, check all boxes that apply. The petitioner should only check the first box if related to the minor(s).

In section four, check all of the boxes that apply.

Complete section seven if petitioning for the guardian of the estate. You’ll have to provide total value of personal property, gross income from all sources, and the combined total of both.

Complete section 8 to the best of your ability to prove why the guardianship is in the minor’s best interest.
“Petition for Guardian GC-210 Step 9”
Section 11 is required if the petitioner is not related to the minor named in item 2 and the petition is not for the estate only.

Section 12 asks the petitioner to attach Declaration Under Uniform Child Custody Jurisdiction and Enforcement Act (Form FL-105/GC-120).

State how many pages are attached, and sign the form. Have your attorney sign the form too.
