When filing a California post-move notice of change of the residence of a conservatee or ward, the document discussed in this article is used if sufficient space was not provided on form GC-080 to document all people to whom a copy of the post-move notice was mailed. Both documents can be obtained from the website maintained by the California Judicial Branch.
