The form discussed in this article must be filed on an annual basis by California attorneys appointed by the court in probate conservatorships or guardianships. The document can be obtained from the website maintained by the California Judicial Branch.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 1: In the first blank box, enter the county in which the court overseeing the case is located, as well as its street address, mailing address, city, zip code and branch name.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 2: In the second blank box, enter your name and state bar number.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 3: Check the box on line 1a if you have had no State Bar discipline imposed since the date of your qualification certification or your last annual certification.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 4: Check the box on line 1b if you have had State Bar discipline imposed since the date of your qualification certification or last annual certification. You must describe the circumstances of this discipline in attachment 1b.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 5: Check the box on line 1c if your professional liability insurance coverage, adequacy of self-insurance or self-insurance program coverage has not changed since the date of your qualification certification or your last annual certification.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 6: Check the box on line 1d if your professional liability insurance coverage, adequacy of self-insurance or self-insurance program coverage has changed since the date of your qualification certification or your last annual certification. You must describe your current circumstances in attachment 2b.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 7: Check the first box on line 3 if your contact information is as stated in your qualification certification or last annual certification. If different, check the second box and enter your firm or employer name on line 3a, your firm or employer address on line 3b, your telephone number on line 3c, your fax number on line 3d, and your email address on line 3e.

California Annual Certification Of Court-Appointed Attorney GC-011 Step 8: On line 4, enter the calendar year and the number of hours of continuing education qualifying for MCLE credit you completed. Document these hours as instructed, then sign and date the form.
