The form discussed in this article is used to allow the personal representative of a California estate to allow or reject a creditor's claim. A copy of the creditor's claim must be attached when filing this document, which can be found on the website maintained by the California Judicial Branch.

If not represented by an attorney, enter your name, address and telephone number. In either instance, entering a fax number or email address is optional.
