INSTRUCTIONS: UTAH ARTICLES OF ORGANIZATION (DOMESTIC LLC)

To form a limited liability company (LLC) in Utah, you must submit an articles of organization form. This form is processed by the Utah Department of Commerce's Division of Corporations & Commercial Code and is available on their website. You must complete it on a computer or typewriter.

Utah Articles Of Organization (Domestic LLC) Step 1: Line one asks for the name of your LLC.

Utah Articles Of Organization (Domestic LLC) Step 3: Line three asks for the name and address of your LLC's registered agent. You should also indicate whether this agent is an individual, a commercial registered agent or business entity. This registered agent must have a Utah street address.

Utah Articles Of Organization (Domestic LLC) Step 5: Line five asks you to give the name and address of every non-member/-manager organizer. Attach additional sheets as necessary. You may include the business address for confidentiality purposes.

Utah Articles Of Organization (Domestic LLC) Step 7: Line seven asks for the names and addresses of all managers. Attach additional sheets as necessary.
