Enter the limited liability company’s name in Article I. The name needs to include the phrase “Limited Liability Company” or “LLC.” The name cannot contain the word “corporation” or “incorporated.” Use the following link to perform a preliminary search on the availability of the company’s name.

In Article II, provide address for the limited liability company’s registered office and office of the agent. List the registered agent’s name in the box below the address.

If the duration of the limited liability company is less than perpetual, provide a date in Article III.
