INSTRUCTIONS: MD APPLICATION FOR SOLE PROPRIETORSHIP AND/OR GENERAL PARTNERSHIP
Use this form to register for a sole proprietorship or general partnership in the state of Maryland. The form will help the state determine if the business needs an identification number with the Department of Assessments and Taxation related to personal property. “MD Application for Sole Proprietorship and/or General Partnership Step 1”
Provide the owners name on the first line. Write the federal ID number on the second line. If the owner does not have an ID number, provide a social security number.
“MD Application for Sole Proprietorship and/or General Partnership Step 2”
List the federal principal business code number on line 3. Indicate the trading name on line 4, the mailing address on line 5, and the business location on line 6.
“MD Application for Sole Proprietorship and/or General Partnership Step 3”
State the nature of business for the SP or GP on line 7. State “Yes” or “No” on line 8 to indicate if the business is currently active. Provide the business’ starting date on line 8, and state “Yes” or “No” on line 8 to answer if the business was started new.
“MD Application for Sole Proprietorship and/or General Partnership Step 1”
If you answer “No” on line 8, provide the name and address of the previous owner on line 9. State whether or not the owner was incorporated on line 10. If you state “Yes,” list the Department ID number of the corporation on line 11if known.
“MD Application for Sole Proprietorship and/or General Partnership Step 4”
Indicate if the business needs a trader’s, construction, other license on line 12. State if the business owns or leases personal property such as furniture, equipment, machinery, and etc. on line 13. If you answer “Yes,” provide the estimated value of the personal property on line 14. If you answer “No” on line 13, declare if you believe you’ll own, lease, or use personal property in the future on line 15.
“MD Application for Sole Proprietorship and/or General Partnership Step 5”
The owners need to provide their signatures at the bottom of the form along with the business’ telephone number. Send the form to the following address:
