INSTRUCTIONS: STATEMENT OF PARTNERSHIP AUTHORITY UPA 303

Use this form in the state of Illinois in order to establish limits of authority for partners. You’re encouraged to consult with an attorney while completing this form.

“Statement of Partnership Authority UPA 303 Step 1”

Provide the name of the partnership in part 1 of this form. In part 2, list the partnership’s Federal Employer Identification Number (FEIN).

Enter the street address for the chief executive office of the partnership in part 3. Include the city, state, and zip code.

Enter the name and address for the registered agent and their office in part 4. Next, list the names and mailing address of all partners or the name and mailing address of the agent that maintains a list of names and addresses of all the partners in part 5.

In part 6, list the names of all partners that are allowed to transfer real property held by the name of the partnership. If additional space is needed, provide a separate sheet.
