INSTRUCTIONS: APPLICATION FOR RESERVATION OF NAME BCA-4.10
This form is required if a corporation is forming in the state of Illinois and wants to reserve its name before filing Articles of Incorporation. If the name is approved, the reservation will remain valid for 90 days.
“Application for Reservation of Name BCA-4.10 Step 1”
Provide the requested name for the corporation in section 1 of this form. State law requires the name of the corporation to have one of the following designations in the name: corporation, company, incorporated, limited, or an appropriate abbreviation.

State the main purpose of the corporation in section 2 of this form.
“Application for Reservation of Name BCA-4.10 Step 3”
Provide the full legal name of the application in section 3 of this form. Enter the address of the applicant in the section 4.
“Application for Reservation of Name BCA-4.10 Step 4”
Section 5 requires a signature from the applicant. If an individual is applying for the name reservation, they need to provide their signature and printed name below the signature. If a corporation is applying, an authorized officer within the corporation needs to provide their signature. Make sure to date the form including the month, day and year.

A filing fee of $25 is required along within this form. The payment needs made by check or money order and made pay to Secretary of State. Send the form to the following address:

If the Secretary of State approves the name reservation, the applicant will receive a certificate of mail. Make sure to attach this form to the Articles of Incorporation.
