INSTRUCTIONS: CERTIFICATE OF AMENDMENT LLPA-1-1.0

Use this form to amend the original Articles of Organization for a domestic or foreign limited liability partnership. Consult with an attorney if you have questions about this form.

“Certificate of Amendment LLPA-1-1.0 Step 1”

The filing party needs to provide their full name and mailing address at the top of this form. The confirmation is sent to the filer’s address.

“Certificate of Amendment LLPA-1-1.0 Step 2”

Provide the current name of the limited liability partnership according to how it appears in records under the Secretary of the State. If the limited liability partnership is changing its name, the changes need to appear in section 2.

Provide the specific text of each amendment in section 2. Changes might include the LLPs change of name, new partners or the elimination of partners, change of address, and more.
