INSTRUCTIONS: APPLICATION FOR RESERVATION OF CORPORATE NAME RN-06

This form is used to reserve the name for a for-profit corporation, nonprofit corporation, or limited liability company attempting to form within the state of Arkansas. Follow these instructions closely.

“Application for Reservation of Corporate Name RN-06 Step 1”

Check one of the two boxes at the top of this document. If the corporation currently has jurisdiction in another state or another country, check the second box. If the corporation wants domestic jurisdiction, check the first box.

“Application for Reservation of Corporate Name RN-06 Step 2”

Check the appropriate box for the type of corporation being formed in the state of Arkansas. If you’re forming a for-profit corporation, check the box. If you’re forming a nonprofit corporation, check the second box. If you’re forming a limited liability company, check the third box.

“Application for Reservation of Corporate Name RN-06 Step 3”

Provide the requested name for the corporation on the line provided. A corporation’s name must contain the word “Corporation,” Incorporated,” “Company,” or “Limited.” An abbreviation will work as well, such as “Corp.,” “Inc.,” “Co.,” or “Ltd.”

Provide the physical address including city, state and zip code where the corporation will operate. Do not provide a PO Box address. The applicant needs to sign and print there name on the bottom of this form. Provide the date of signature as well. Knowingly providing false information on this form is punishable with a $100.00 fine and/or 30 days of imprisonment in Arkansas.

The filing fee for a mailed form is $25.00. A form submitted electronically requires a filing fee of $22.50. If you’re mailing the form, make the check payable to the Secretary of State. Send the form to the following address:

“Application for Reservation of Corporate Name RN-06 Step 5”

The reservation will last 120 days or until the corporation is officially established in Arkansas. If the corporation is not established within 120, the for-profit and non-profit corporation reservations cannot be renewed. The reservations for an LLC can be renewed once for an additional 120 days.
