INSTRUCTIONS: ARKANSAS APPLICATION FOR CHILD SUPPORT SERVICES FOR CHILDREN RECEIVING ARKIDS 1ST

Arkansas custodial parents whose children are enrolled in the state ARKids 1st Step program may file the application discussed in this article if they have not been fully paid court-ordered child support. This document can be obtained from the website of the Arkansas Department of Finance and Administration as part of a larger packet containing instructions. The actual application begins on the ninth page.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 1: The first page of the application requires you to provide information about your children. In the first column of the table provided, enter their names.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 2: In the second column, enter their sex.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 3: In the third column, enter their Social Security number.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 4: In the fourth column, enter their Social Security number.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 5: In the fifth column, enter the county and state of their birth.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 6: In the sixth column, write whether or not a paternity acknowledgment was signed at the hospital.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 7: On the second page, provide all information required about yourself.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 8: On the third page, provide all information required about the noncustodial parent.

Arkansas Application For Child Support Services For Children Receiving ARKids 1st Step 9: On the fourth page, provide any other information you believe is important to share, as well as information about your legal status with the noncustodial parent and medical insurance information.
