INSTRUCTIONS: ARKANSAS ADDITIONAL OWNER INFORMATION FORM (Form ST-1-A)
Arkansas businesses filing for a sales and use tax permit by using a form ST-1 may not have sufficient space on the form to document all of their owners. If this is the case, a form ST-1-A allows you to document up to four additional owners on one page. Both documents can be obtained from the website of the Arkansas Department of Finance and Administration.
Arkansas Additional Owner Information Form ST-1-A Step 1: The information required to document every owner is the same in each section. In the first box, write the first name of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 2: In the second box, write the middle initial of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 3: In the third box, write the last name of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 4: In the fourth box, write the Social Security number of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 5: In the fifth box, write the title of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 6: In the sixth box, write the percentage of the ownership allotted to the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 7: In the seventh box, write the street mailing address of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 8: In the eighth box, write the city of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 9: In the ninth box, write the state of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 10: In the tenth box, write the zip code of the owner, officer or partner.
Arkansas Additional Owner Information Form ST-1-A Step 11: When completed, this form must be submitted along with your form ST-1. Note that there is a $50 non-refundable fee for submitting this application. Failure to submit payment of this fee at the time of application may result in the delay of the issuance of a sales and use tax permit. Mail the form to the address given at the top of the first page of form ST-1.
