Within 30 days of the sale or transfer of an Arizona motion picture credit, the transferor must file a form 334-O. This document can be obtained from the website of the Arizona Department of Revenue.

For each transferee, provide their name, tax identification number, address, and the name and telephone number of their authorized contact person, as well as the credit amount. Total the credit amount on line 5.
