INSTRUCTIONS: ARIZONA MOTION PICTURE CREDITS (Form 334)

Arizona form 334 is used to document motion picture credits being claimed an individual, corporations and S corporations. This document can be obtained from the website of the Arizona Department of Revenue.

Arizona Motion Picture Credits 334 Step 1: If not filing for the calendar year pre-printed on the form, enter the beginning and ending dates of your fiscal year.

Arizona Motion Picture Credits 334 Step 2: Enter your name as it appears on form 140, 140PY, 140NR, 140X, 120, 120A, 120S, 120X or 165.

Arizona Motion Picture Credits 334 Step 3: Enter your Social Security number or employer identification number.

Arizona Motion Picture Credits 334 Step 4: Part I concerns the individual who will maintain records of expenditures in Arizona. Enter their name, address and telephone number.

Arizona Motion Picture Credits 334 Step 5: Part II requires you to list postapproved productions. On lines 1 and 2, enter the allocation year in column (a)(a)1, the post-approval date in column (a)(a)2, the post-approval number in column (a)(a)3, and the approved credit amount in column (b)(b). If you are documenting more than two completed productions, attach additional schedules and list their aggregate total on line 3.

Arizona Motion Picture Credits 334 Step 6: Enter the total of lines 1 through 3 on line 4.

Arizona Motion Picture Credits 334 Step 7: Part III should only be completed by S corporations. On line 5, indicate whether you are claiming the credit or passing it through to shareholders. If the latter, you must complete lines 6 through 8 separately to document each shareholder.

Arizona Motion Picture Credits 334 Step 8: Enter your name and tax identification number at the top of the second page.

Arizona Motion Picture Credits 334 Step 9: Part IV concerns each partner's share of the credit and must be completed separately for each partner. Enter the name of the partner on line 9.

Arizona Motion Picture Credits 334 Step 10: Enter the partner's tax identification number on line 10.

Arizona Motion Picture Credits 334 Step 11: Enter the partner's share of the available credit on line 11.

Arizona Motion Picture Credits 334 Step 12: Part V concerns the available credit carryover. Complete the table as instructed.
