INSTRUCTIONS: ARIZONA TRANSFEREE NOTICE OF TRANSFER OF MOTION PICTURE CREDITS (Form 334-E)

Following the transfer of any motion picture credit in Arizona, the transferee must submit a form 334-E to the Department of Revenue no later than 30 days after the transaction. This document can be obtained from the website of the Arizona Department of Revenue.

Arizona Transferee Notice Of Transfer Of Motion Picture Credits 334-E Step 1: Enter the processing fee enclosed where indicated. To compute the processing fee, multiply the amount of the transfer by 1%. Round the resulting product to the nearest dollar. If it is less than $200, write that amount. If $200 or more, write $200.

Enter their name, tax identification number, number and street or PO box number, city or town, state and zip code. Additionally, provide the name of an authorized contact person and their telephone number.

Arizona Transferee Notice Of Transfer Of Motion Picture Credits 334-E Step 3: Section B concerns the motion picture company in question. Enter their name, tax identification number, number and street or PO box, city or town, state and zip code.

Arizona Transferee Notice Of Transfer Of Motion Picture Credits 334-E Step 4: In the first box of Section C, enter the date of the transfer.

Arizona Transferee Notice Of Transfer Of Motion Picture Credits 334-E Step 5: In the second box of Section C, enter the amount of the transfer rounded to the nearest whole dollar.
