INSTRUCTIONS: ARIZONA MOTION PICTURE CREDITS (Form 334)

To document Arizona motion picture credits, a form 334 should be filed with the state Department of Revenue. This document can be obtained from the website maintained by the Arizona Department of Revenue.

Arizona Motion Picture Credits 334 Step 1: Enter the beginning and ending dates of your fiscal year at the top of the first page if not filing for the calendar year pre-printed on the form.

Arizona Motion Picture Credits 334 Step 2: Enter your name as it appears on form 140, 140PY, 140NR, 120, 120A, 120S, 120X or 165.

Arizona Motion Picture Credits 334 Step 3: Enter your Social Security number or employer identification number.

Arizona Motion Picture Credits 334 Step 4: On the first blank line of Part I, enter your name.

Arizona Motion Picture Credits 334 Step 5: On the second blank line, enter your telephone number.

Arizona Motion Picture Credits 334 Step 6: On the right, enter your complete address.

Arizona Motion Picture Credits 334 Step 7: Part II requires you to list postapproved productions.

Arizona Motion Picture Credits 334 Step 8: Part III concerns S corporation credit election and shareholder's share of credit. Enter the allocation year.

Arizona Motion Picture Credits 334 Step 9: Check the box next to the first statement if claiming a credit related to costs shown on line 4 of Part II.

Arizona Motion Picture Credits 334 Step 10: Check the box next to the second statement if passing the credit through to your shareholders.

Arizona Motion Picture Credits 334 Step 11: If passing the credit through to shareholders, complete lines 6 through 8 separately for each shareholder. Enter each shareholder's name on line 6, their taxpayer identification number on line 7 and their share of the available credit listed on line 4 on line 8.

Arizona Motion Picture Credits 334 Step 12: Part IV concerns each partner's share of the credit. Complete lines 9 through 11 separately for each partner. Enter their name on line 9, their tax identification number on line 10, and their share of line 4 on line 11.

Arizona Motion Picture Credits 334 Step 13: Part V concerns the available credit carryover for motion picture production costs. In order to complete lines 14 and 17, if applicable, you will need to complete form 334-1, which is located on the third page.
