Arizona Qualified Employees Of Healthy Forest Enterprise 332-1 Step 1: On line one, enter the name of the employee.

Arizona Qualified Employees Of Healthy Forest Enterprise 332-1 Step 2: On line two, enter the employee's tax identification number (TIN).

Arizona Qualified Employees Of Healthy Forest Enterprise 332-1 Step 3: On line three, indicate with a check mark whether the employee was a resident of Arizona on the date on which they were hired.
