INSTRUCTIONS: ARIZONA RENTER'S CERTIFICATE OF PROPERTY TAXES PAID (Form 201)
Renters in Arizona whose rent includes a portion to cover property taxes can obtain a certificate from their landlord stating this is the case in order to obtain a
property tax credit. This form 201 can be found on the website of the Arizona Department of Revenue.

Property Taxes Paid 201 Step 1: Part I should be completed by you. Enter your first name and initial, your last name and your
Social Security number.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 2: If you are married, provide your spouse's name and Social Security number as well.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 3: Give the form to your landlord, who will complete Part II. On line 1, they should enter their name.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 4: On line 2, the landlord should enter their number and street address.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 5: On line 3, the landlord should enter their city, state and zip code.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 6: On line 4, On line 4, if applicable, the landlord should enter the
business name of the rental property.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 7: On line 5, the landlord should enter the number and street address of the rental property.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 8: On line 6, the landlord should enter the city, county, state and zip code of the rental property.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 9: The landlord should check the box next to all applicable statements on lines 7 through 9.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 10: The landlord should enter the property
tax factor on line 10, sign and date line 11, then print their name and phone number on line 12.
Arizona Renter's Certificate Of Property Taxes Paid 201 Step 11: After the certificate has been returned to you, enter your total rents paid for the calendar year on line 13, enter the property tax value from line 12 in line 14, then multiply lines 13 and 14. Enter the resulting product on line 15. Transfer this result to line 13 of form 140PTC.
