INSTRUCTIONS: ARIZONA APPLICATION FOR FILING EXTENSION FOR FIDUCIARY RETURNS ONLY (Form 141 AZ EXT)
Fiduciaries in charge of an Arizona estate of trust may file a form 141 AZ EXT if they need time past the due date to complete their state fiduciary return forms. This does not need to be submitted if you have obtained a federal extension. This form will grant a five month extension to file. If you do not submit a payment with this application, a financial penalty and interest will apply. This form is found on the website of the Arizona Department of Revenue. This article concerns the form filed for calendar year 2011.

On line two, enter the name and official title of the fiduciary. On lines three and four, give the full address of the fiduciary and their daytime telephone number, including the area code. If you are a first time Arizona income tax return filer, check box one.

If not, check the box next to "Fiduciary Fiscal Year Filers" and give both the taxable year's end date and the extended due date five months after this.

You are permitted to estimate this amount. On line two, enter the total of any estimated tax payments. Subtract this from line three to determine your tax balance on line three. On line four, enter the size of the payment being made, if any.
