INSTRUCTIONS: EMANCIPATION OF A MINOR PETITION PACKET
“Emancipation of a Minor Petition Packet Step 1”
You must meet all qualifications. You cannot use the forms in this packet if you’re a ward of the court or you’re legally married.
“Emancipation of a Minor Petition Packet Step 2”
You need to fill out all of the following forms in this packet in black ink:
1. Juvenile Emancipation Information Sheet- you need to complete everything except the case number. Once the clerk stamps the case number, you’ll use this case number on all other forms.
2. Motion to Seal Court Record for Reasons of Confidentiality- you need to complete this form if you wish to keep the case information private.
3. Petition for Emancipation of a Minor- click on the following link for instructions for this form: http://www.azcourts.gov/Portals/34/Forms/Emancipation/PetInstructions.pdf
4. Consent to Emancipation of a Minor- this form is not required, but a parent who consents can complete this form and sign it in front of the Clerk of Court or Notary Public.
Once you submit the forms, the court will provide you with a date, time and location of the hearing by mail.

You need to acquire two packets—one set of originals and one set of copies. Copies should include the following:
· 1 copy of Petition for Emancipation of a Minor
· 1 copy of Consent to Emancipation of a Minor if a parent or guardian signed
· 1 copy of Motion to Close Court Record for Reasons of Confidentiality

Take the original forms and copies to your local Clerk. You can find contact information for your local Clerk of Court within the government pages of the telephone directory.

You’ll have to pay your filing fee. If you can’t pay the filing fee at this moment, you can fill out an application for a deferral. A judge may also grant a waiver for the fees.

Once the petition is filed, the court will schedule a hearing within 90 days. The court will notify all parties that need to attend the hearing and provide a date and time.
