When an Arizona biological parent gives up custodial rights to their child and allows them to be adopted, they use a form AN-010-FF in order to officially waive their right to notification of any hearings concerning dependency, severance and adoption of their child. This document can be obtained on the website maintained by the Arizona Department of Economic Security.
