INSTRUCTIONS: AL PETITION FOR CHANGE OF NAME (Jefferson County)

When filing this form with the Probate Court, prepare to pay a filing fee of $65 to $80 by cash or money order. You’ll also need the following identification:

· Alabama driver’s license or photo I.D.

· Birth certificate

· Marriage and/or divorce certificate (if necessary)

· Two proofs of residency in Jefferson County (such as utility bill, voter registration, deed, etc)
