INSTRUCTIONS: ALABAMA INDIVIDUAL INCOME TAX RETURN RESIDENTS & PART-YEAR RESIDENTS 2011 (Form 40)
Alabama residents who are full time or part-year can use a form 40 to file their state tax returns. This form can be found on the website of the Alabama Department of Revenue.
Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 1: At the top of the page give your name and address. If filing a joint return, enter your spouse's name.
Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 2: Questions one through four ask you to indicate your filing status and attendant exemption.
Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 3: Question 5 requires you to detail your wages, separating your income and income tax withheld. Give the name and address of each employer.
Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 4: Enter any interest or dividend income in response to question 6. Calculate all other income in Part I on the second page and enter the sum in response to question 7.

Enter the sum in response to question 8 Complete Part II on the second page to determine your adjustments and enter the result in response to question 9. Subtract the result from the response to question 8 and enter the result in response to question 10.
Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 6: Questions 11 through 15 concern deductions. You will need to complete Part III on the second page if claiming dependents.

Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 8: Questions 22 through 27 adjust your balance based on payments already made.
Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 9: Calculate the amount owed in response to questions 28 and 29. Questions 30 and 31 concern those who have overpaid, question 32 concerns deductions, and question 33 concerns any refund due.
Alabama Individual Income Tax Return Residents & Part-Year Residents 2011 40 Step 10: Complete Part IV on the second page. Sign and date the form. Give your daytime telephone number and occupation.
