INSTRUCTIONS: ALABAMA CERTIFICATE OF DIVORCE

An Alabama Certificate of Divorce should be submitted by the plaintiff's petitioner in the applicable circuit court as part of the proceedings. It should be filled out by either the legal representative or the plaintiff themselves if undergoing self-representation. The purpose of the form discussed here is to summarize the information relevant to a divorce which has been granted.

Alabama Certificate Of Divorce Step 1: In box one, the date that a divorce decree was issued should be entered.

Alabama Certificate Of Divorce Step 2: Boxes 2-13 are for information concerning the male in the marriage. Their full name, address, date of birth, address, Social Security number, race and last level of education completed should all be included.
