INSTRUCTIONS: ALABAMA APPLICATION FOR BENEFICIARY PAYMENT DEFERRED RETIREMENT OPTION PLAN (Form RSA DROP BEN) Surviving beneficiaries of deceased enrolled participants in Alabama's Teacher Retirement System (TRS) or Employee Retirement System (ERS) use a form RSA DROP BEN to apply for payments of their benefits. This document is found on the website of the Retirement Systems of Alabama. Alabama Application For Beneficiary Payment Deferred Retirement Option Plan RSA DROP BEN Step 1: At the top right-hand corner, indicate with a check mark whether the deceased was enrolled in TRS or ERS. Alabama Application For Beneficiary Payment Deferred Retirement Option Plan RSA DROP BEN Step 2: On the first three lines of Part I, give the deceased's name, Social Security number, dates of birth and death and employing agency. The first part of the form should only be completed by spouses of the deceased. If applicable, check Part A if you wish to receive payment through a lump sum. Check Part B if you wish to have the entire fund rolled over to a trustee. Indicate with a check mark what type of account the money should be transferred into. A trustee official should sign and date this section. Alabama Application For Beneficiary Payment Deferred Retirement Option Plan RSA DROP BEN Step 6: If you are a non-spousal beneficiary, at the bottom of the first page indicate whether you wish to receive a lump sum payment or have a direct rollover of the funds into a qualified account. If the latter, give all requested trustee information on the next page. Part IV should be completed by the deceased's employing agency.
